You must accept these Terms and Conditions for Use to access
"MyJeffersonHealth" online.
MyJeffersonHealth is an internet application that allows a
patient to have secure web based access to elements of their Jefferson
electronic medical record. MyJeffersonHealth allows a
secure, convenient communication channel with Jefferson for non-urgent
messages.
Use of the MyJeffersonHealth service
is optional.
Your login username and password are key in keeping your
information confidential and protecting your personal information. It is your
responsibility to keep your username and password confidential. Your username
and password are only known by you and can be changed online at any time by
you. Your password is not known by anyone at Jefferson.
MyJeffersonHealth Terms and Conditions for Use: Your Responsibilities
Please review the following Terms and Conditions for Use. You
must observe the Terms and Conditions for Use described below when using MyJeffersonHealth.
I AGREE
THAT:
1) I
will not share my username and password with anyone.
2) I
will not send any messages through MyJeffersonHealth
that require urgent or emergent attention. I will call 9-1-1 in the case of an
emergency. I understand that turnaround time for responses is usually two (2) business
days, but may vary by individual physician or practice. Jefferson does not
guarantee that I will receive a response to any particular message. If I have
not received a response within a reasonable time and feel the message must be
addressed, I should call my physician or my physician's office.
3) I
will be notified by e-mail when there is new medical information to be viewed
on MyJeffersonHealth. This means that any person with
access to my e-mail will be able to see this notification. This could include
my spouse, dependent, employer or anyone else that can access my email account.
Although no private medical information will be included in the e-mail, the
notification that new medical information is available by accessing MyJeffersonHealth may be information that I would not want
others to know. Thus, I should take this into consideration when providing an
e-mail address. I am responsible to make sure that my contact information is
accurate and up-to-date.
4) MyJeffersonHealth messages may be viewed by other health
professionals, administrators, or support staff who work with my physician and
his/her practice.
5) Neither
MyJeffersonHealth nor any other application available
through this site is a substitute for appropriate and timely contact with my
physician(s). This site is not a substitute for an office visit or consultation
with my physician(s). I understand that I should never change or stop any
course of treatment prescribed by my physician(s) without first consulting him
or her.
6) Not
all of my Jefferson physicians and providers use MyJeffersonHealth,
so they may not be aware of the information that is in my MyJeffersonHealth.
I understand that I may not be able to communicate electronically or view
information from all of my Jefferson providers.
7) There
may be some information that my Jefferson physicians believe should not be
placed in my MyJeffersonHealth record. To obtain a
complete copy of my Jefferson medical records, I should follow the Jefferson
policy for requesting medical records.
8) All
communications through MyJeffersonHealth will be in
regard to my own medical condition(s). The contents of any messages will be
stored in my permanent Jefferson medical record.
9) I
understand that I may only ask for advice about me through MyJeffersonHealth.
I understand that asking for advice on behalf of another person could
potentially be harmful and is a violation of these Terms and Conditions for
Use. JEFFERSON DOES NOT ASSUME ANY RESPONSIBILITY FOR MEDICAL INFORMATION
OR SERVICES USED BY A PERSON OTHER THAN ME.
10) I
may receive messages, test results and other communications delivered via MyJeffersonHealth. I understand that I may receive test
results before my Jefferson provider contacts me or reviews the test results.
If I have questions or concerns regarding test results that were made available
to me via MyJeffersonHealth, I should contact my
physician's office.
11) I
am bound by all of these Terms and Conditions for Use. I know that Jefferson
may modify these Terms and Conditions for Use. If changes are made to these
Terms and Conditions for Use, I will be notified and required to review the
modifications. The modifications shall be effective immediately upon posting of
the modified version.
12) MyJeffersonHealth is a communication service offered as a
convenience to me. My MyJeffersonHealth may be
deactivated by me. I understand that Jefferson may also deactivate my access to
MyJeffersonHealth for any reason at any time,
including if I do not abide by these Terms and Conditions for Use.
13) I
understand that MyJeffersonHealth provides me online
access to elements of my Jefferson medical record and that I am solely
responsible for any sharing of its content that I intentionally or
unintentionally communicate to others.
14) Messages
sent using my MyJeffersonHealth may be sent to
members of my physician's care team or other members of Jefferson staff to
appropriately address my requests. Therefore, I understand that the use of MyJeffersonHealth may not be appropriate for very sensitive
information that I wish to discuss directly or in person with my physician or
Jefferson provider.
15) I
will not use inappropriate, profane, or threatening language in my messages
sent using my MyJeffersonHealth.
16) This Web
site may contain links to Web sites operated by other parties, such as Fitbit,
Apple HealthKit, and Red Brick. The links are
provided for Individual's convenience only. Neither Jefferson Health nor Epic
controls such Web sites and Jefferson Health is not responsible for the content
and performance of these sites. The inclusion of links to other Web sites does
not imply any endorsement of the material on the Web sites or any association
with their operators. Jefferson Health does not operate, control, or endorse
any information, products, or services provided by third parties through the
Internet. Use of other sites is strictly at
Individual's own risk, including, but not limited to, any risks associated with
destructive viruses. Individual is responsible for viewing and abiding by the
terms and conditions of use and the privacy statements of the other Web sites.
17) This
Web site may contain links to other Web sites operated by Jefferson Health. Use
of such other Jefferson provided Web sites is subject to the terms provided
with the other Web sites.
18) Some
of the material on the Jefferson Health site is provided by third parties and
Jefferson Health shall not be held responsible for any such third-party
material. Jefferson Health disclaims any responsibility for or liability
related to such third-party material and Individual uses of such third-party
material. Any questions complaints or claims related to any product should be
directed to the appropriate third party.
19) MyJeffersonHealth is provided by Jefferson
Health on an "AS IS" basis. Jefferson Health makes no representations
or warranties of any kind, express or implied, as to the operation of its site,
or the content, products and/or services included therein. To the fullest
extent permissible by applicable law, Jefferson Health and Epic disclaim all
warranties, express, or implied, including, but not limited to, implied
warranties of merchantability, fitness for a particular purpose, title, and/or
infringement.
20) The
information collected in MyJeffersonHealth may be
maintained indefinitely as the software does not allow for deletion at this
time. If you chose to use MyJeffersonHealth please be
aware that Jefferson may:
•Store a copy of a picture on your device if
you choose to add a picture to your profile.
•Create encrypted identifiers to identify
target healthcare providers for HealthKit data, if
you are using Apple HealthKit.
•Temporarily store your personal information
in memory or on the device while you use our Applications.
•Share some limited amount of information for
purposes of troubleshooting and error correction directly or indirectly with
Epic with your consent.
•In addition, in order to provide you certain
features, our Applications may request information from servers and systems
owned or operated by Epic and those servers and systems may record technical
information about that request such as an IP address.
21) If you are a data subject as
defined by the General Data Protection Regulation of the EU, you have a number
of rights and can do any of the following by contacting Jefferson Health via
email at privacyoffice@jefferson.edu
•Request a copy your data;
•Request that Jefferson Health change
incorrect or incomplete data it has about you;
•Request that Jefferson Health stop
processing your data; and
•Express any concerns or objections you have
about Jefferson Health's use of your data.
Please note that if you contact us to assist you, for
your safety and ours we may need to authenticate your identity before
fulfilling your request.
